Appointment requests submitted through our online tool are processed using Microsoft 365 services, which we use under a signed Business Associate Agreement consistent with HIPAA requirements. Information you submit is directed to our staff for the purpose of confirming and scheduling your appointment.

We retain appointment request information for as long as reasonably necessary to manage scheduling and to comply with our recordkeeping practices.

Your Choices

You may contact us at hello@rootandbloom.net to ask questions about information you've submitted through our website, or to request that we delete an appointment request that has not yet resulted in a scheduled appointment.

Our appointment request tool is not intended for use by children under the age of 13 without the involvement of a parent or guardian. If you are seeking care for a minor, a parent or guardian should complete the request on their behalf.
